Change of Address Form (online fillable) This form is used to change an address or contact information. Submit this form, along with a check or money order, to the National Office when paying dues for multiple members of a chapter by check and not online through their member profiles.

Request for an International Prayer Partner (online fillable) Request for an International Prayer Partner (downloadable pdf) This form is used by Daughters chapters, or Daughters at Large, in the United States requesting to become prayer partners with chapters in other countries.
Report of Chapter Disbandment (online fillable) A Chapter wishing to disband may do so after consultation with their clergy or priest-in-charge and the diocesan officers. Use this form to report the chapter's disbandment to the National Office. Also be sure to return the Charter, Transition Forms for each Daughter, crosses of any Daughter not becoming at Large, and any remaining funds. This form is used to order an annual subscription to the Royal Cross.
