Forms for Senior Daughters

Alternate Address Form
This form is used to report different addresses for different times of the year. This form will ensure a Daughter receives her copy of The Royal Cross at both locations.

Application for Dues Relief Status
This form is used on behalf of a Daughter who is in deteriorating health, to apply for relief of her obligation to pay dues. The Daughter must meet all the requirements listed on the form. It must be signed by her clergy and chapter president, or Diocesan or Provincial president if she is a Daughter at Large. For more information about the policy, click here.

Application for National Dues Assistance

This form is used to apply for Dues Assistance. Members are required to read and abide by the Dues Assistance Policy of to receive dues assistance. For information about the policy, click here.

Application for Reinstatement

This form is used when a Daughter, who after two years of not paying her annual dues, needs to be reinstated. A Daughter must observe a period of renewal and study, complete the application with all required signatures, and submit the form to the national office with the current year's dues and a nominal reinstatement fee.

Application for Senior Membership
After completing discernment, this form is used by prospective members to apply for membership in The Order of the Daughters of the King^®. The completed form should be given to the study leader who will place the new member order using the National Order Form (listed below).

Chapter Transition Form

This form is used when contributing to: the four funds of The Order; the purchase of crosses for The Order; and dues financial assistance for other Daughters. 

Disposition of Cross Card
The Disposition of Cross Card is used by a Daughter to convey her wishes as to what should be done with The Order's cross upon her passing.

Individual Dues Payment Form

Submit this form, along with a check or money order, to the national office when paying dues by check and not online through the member profile.

This form is used by Daughters, chapters, or Daughters at Large, in the United States requesting to become prayer partners with chapters in other countries.

Report of Chapter Disbandment (online fillable)
Report of Chapter Disbandment (downloadable PDF)

A chapter wishing to disband may do so after consultation with their clergy or priest-in-charge and the diocesan officers. Use this form to report the chapter's disbandment to the national office. Also be sure to return the charter, Transition Forms for each Daughter, crosses of any Daughter not becoming at Large, and any remaining funds.
