Application for Junior Membership
This form is used by a Junior Directress to apply for membership in The Order of the Daughters of the King® for girls and young women ages 7-21.
Application for Senior Membership
This form is used by a transitioning Junior Daughter to apply for membership in The Order as a senior Daughter. This form is also used by prospective members to apply for membership in The Order of the Daughters of the King®.
Change of Address Form (online fillable)
This form is used to change an address or contact information.
Individual Dues Payment Form
Submit this form, along with a check or money order, to the National Office when paying dues by check and not online through the member profile.

To be used when Directresses contact information changes or when Directresses change or when a Junior Chapter becomes inactive.
National Order Form
This form is used to order Crosses, supplies, brochures, booklets, and literature.
Report of Chapter Disbandment (online fillable)

A Junior Chapter wishing to disband may do so after consultation with their clergy and senior chapter president. Use this form to report the chapter's disbandment to the National Office. Also be sure to return the Charter and Transition Forms for each Junior Daughter especially if she is becoming at Large.

This form is used to report a: change of name; change of address or contact information; transfer to new chapter; member becoming a Daughter at Large; resignation; member’s death.
Acknowledge, Waiver and Release from Liability and Hold Harmless Form
This form is used by Junior Directresses to obtain permission for her Juniors to participate in events and activities outside of the parish.
The Medical Release Form
This form is used by Junior Directresses to obtain permission to provide medical care to her Juniors in the event of an emergency.
The Medical Information Form
This form is used by Junior Directresses to collect information about any medical conditions and/or allergies her Juniors may have and who to contact in the event of an emergency.
